Alumni & Aftercare Services

Aftercare planning is provided for all treatment clients across the agency’s continuum of care. Adelphoi has been offering aftercare and alumni services throughout its entire history. In 1993, Adelphoi combined its already existing aftercare program and added the outcomes component, centralizing these services with the inception of the Outcomes and Alumni Office.

Effective application of care stresses individualization, progression, and collaboration as key planning factors in every youth’s successful transition. Client- specific plans, which align with the MacArthur Foundation Models-for-Change Initiative, include the following components:

  • Education
  • Employment
  • Transfer of Skills (primarily impulse control and problem solving)
  • Family Involvement
  • Peer selection
  • BARJ
  • Medical (coverage and prevention)
  • Specialized treatment concerns (i.e. substance abuse, PTSD, gang involvement, and sexual issues)
  • Adelphoi is dedicated to assisting youth in making a successful transition from placement. If you are an alumnus of Adelphoi, we can help you through ongoing support, referrals to community services, and even financial assistance.

Please call our Office of Alumni Services if:

    • You have moved or have a new phone number and would like to continue receiving our mailings.
