Aftercare Planning

  • Male and Female Youth, Ages 0-21

Aftercare Planning is provided for all youth across Adelphoi Village’s Continuum of Care. Youth begin Aftercare Planning the day they are admitted to Adelphoi Village.   • Education
  • Employment
  • Transfer of Skills
  • Family Involvement
  • Peer Selection
  • Balanced and Restorative Justice
  • Medical (coverage and prevention)
  • Specialized Treatment Concerns

Aftercare Group is also offered to youth, providing them with an extensive opportunity to understand and practice their learned treatment skills. Each client attends a basic group two months prior to discharge and an advanced group one month prior to discharge. Discussion, small group activities, and hands-on exercises engage group members to anticipate post-discharge barriers as they think, plan and problem-solve their way through skills aimed at reducing the challenges they will face after release from the program.
